COVID-19 POLICY

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

 

Katana Spa, Inc., d/b/a Katana Spa has put in place preventative measures to reduce the spread of COVID-19; however, Katana Spa, Inc., d/b/a Katana Spa cannot guarantee that you will not become infected with COVID-19. Further, visiting Katana Spa could increase your risk of contracting COVID-19.

 

By entering our premises, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you may be exposed to or infected by COVID-19 by visiting Katana Spa and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 at Katana Spa may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Katana Spa, Inc., d/b/a Katana Spa employees, volunteers, and other Spa and Salon guests and their families.

 

By entering our premises, you voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with visiting Katana Spa. Upon entering our premises, you hereby release, covenant not to sue, discharge, and hold harmless Katana Spa, Inc., d/b/a Katana Spa its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after visiting Katana Spa, Inc., d/b/a Katana Spa.

 

In an effort to reduce the risk of COVID-19 exposure and to help prevent the spread of the virus, you may not enter the Spa if you have had any recent exposure to the COVID-19 virus. Masks are required when you enter the Spa and must remain on until directed to remove by a member of our staff.  Respect the space of those around you and allow from proper social distancing, as best we can, while inside the Spa.

 

If any of the following apply to you, you may not enter the Spa, kindly call us to reschedule your appointment:

  • If you do not feel well, for any reason, including but not limited to fever, cough or shortness of breath within the past 14 days.

  • You have had close contact with an individual infected with COVID-19 within the last 14 days.

  • You have had close contact within the last 14 days with individuals suspected of being infected with COVID-19, including individuals exhibiting COVID-19 symptoms.

  • You have traveled outside of Massachusetts.
